Outstanding Clinical Care Award
Recognising Healthcare Professionals leading the way in providing excellence in care for our Trisomy families.
Candidates can be from consultants, clinicians, nurses, midwives, geneticists, doctors, paediatricians
Outstanding Compassionate Support Award
Recognising those who have gone above and beyond to provide compassionate support to families who have experienced a Trisomy related bereavement.
Candidates can be from hospices, palliative care teams, carers, support workers, family members.
SOFT Community Hero
Recognising those within our SOFT community who have reached out to a fellow SOFT member with support and guidance during their trisomy experience.
Candidates can be a family or a person you have met through SOFT UK.
